Privacy Policy
There is a Privacy Policy in place in accordance with the Personal Health Information Protection Act (PHIPA). This policy helps to ensure that I, as a personal health custodian, (ie. health care practitioners) collect, use, and disclose your personal health information in a responsible manner. Personal health information includes oral or written information that relates to your physical or mental health (including family history, health conditions and services received) and your personal information (eg. age, phone number, family status, etc).
Who can see your personal health information?
When you seek health care here, it is implied that you have expressed consent that your personal health information may be shared with other health care providers for the purpose of treatment. This also includes substitute therapists who may take over treatments in case of illness, emergency, or vacation. If you do not want your personal health information shared with any other health care providers, please inform me as soon as possible.
Occasionally third parties may request your information, such as insurance companies or employers, however, I must receive your written permission in order to release the information.
When your consent is not required:
There are certain instances in which your information may be used or shared when consent is not required, just a few examples are: to eliminate or reduce risk of serious bodily harm; to a medical officer of health or public health authority; for the purpose of a legal proceeding or complying with a court order.
Storage of information:
Your information will be retained in a secure method. Paper files are either under supervision or in locked cabinet. Any personal information on computer (name, number, email address) will always be password protected. As required by the College of Massage Therapists of Ontario your information will be kept on file for ten years after your last visit. You have the right of access to your own personal health information with a few exceptions, such as information relating to legal proceedings or investigations.
